About The Role Hours: 36 Per week (Monday – Friday Only) As a practitioner, you will be delivering a Portage-based home-visiting early intervention service to children under 5 with special educational needs or disabilities and assisting with the running of play sessions in Southwark. A positive and caring attitude and a desire to make a difference. |

Working or supporting disabled children. |
- Building partnerships with parents
- Making regular weekly visits to family homes
- Working with the Coordinator or another member of the team runs a weeklyPortage Group
- Managing own casework
- Setting our intervention plans to include – weekly activities, reviewing progress andproducing relevant written reports
